"Pre Yom Kippur Mussar" by Rabbi Nissan Kaplan, shlita

Here's a brief overview of the Pre Yom Kippur Mussar shiur to make it easier to follow.
R. Kaplan has a kabala of this method from Rav Yechezkel Levenstein zt'l, who was the Mashgiach of the Mir Yeshiva and later on Ponovezh.
  1. make a list of all problem areas.
  2. narrow down to no more than 5 of the most important ones that could change your life.
  3. Find a resolution in each of the 5 (as Rabbi Kaplan explains).
  4. Each resolution is like a brick wall and must be kept. Find something then think and "cut the corners off" until you have something doable (as Rabbi Kaplan explains).
  5. Prepare yourself for the various traps of the yetzer hara (see the shiur).

 
It's probably a good idea also to reread (or say from memory) the resolutions every day.
